For professionals

The first step in the referral process is a visit to the child’s family physician or pediatrician. Following the visit, the referring physician should submit a completed referral form to the clinic’s intake coordinator. Please note that not all referrals are accepted as appropriate for this clinic, and some may be redirected to community-based resources. If the referral is accepted for assessment, an information package that includes a questionnaire and consent forms will be sent to the family.
