	For professionals Referrals A written referral outlining the clinical symptoms and reason for the referral is required from the referring physician. At a weekly meeting, clinic team members review the referrals for appropriateness and to determine priority. The families accepted to the program are assigned to one of the psychiatrists and to other team members as appropriate. Families not assigned to the clinic are redirected to appropriate services in their home community. Download the referral form here. Completed referral forms are to be faxed to 604-875-2099.
