Referrals

This information is required to refer a patient to any of the Children’s Heart Center services. For further information, please contact Children’s Heart Centre at 604-875-2120.

 

Cardiovascular and thoracic surgery

 
Emergency referrals
  • Phone the surgical team (Dr. Jacques LeBlanc & Dr. Andrew Campbell) directly at 604-875-3165
Elective outpatient referrals
  • Fax a completed referral letter to Children’s Heart Centre, Surgical Office, at 604-875- 3159

Urgent outpatient referrals

Request for an electrocardiogram

Request for an echocardiogram requires a pediatrician referral

Request for a Holter monitor requires a pediatrician referral

Patient referrals can be faxed with the patient demographics and reason for referral along with any testing they may have had elsewhere to the clinic. The clinic secretary will contact the referring physician’s office with an appointment date and time.
