Changes in today’s health care environment have led Children’s health care providers to consider alternative care delivery systems. Children’s Heart Centre was faced with the challenges of providing cost-efficient care, achieving desired child and family outcomes, and meeting provincially mandated decentralization of health care. In an effort to rise to these challenges, the Community Partnership in Pediatric Cardiology was developed and implemented in 1994.
The program allows children with cardiac conditions to receive specialized cardiac care in their own community by a multidisciplinary team from Children’s Heart Centre. Infants and children with a suspected non-acute cardiac condition or those with an established diagnosis of congenital heart disease requiring routine follow-up care are eligible for referral to the program.
The Children’s Heart Centre team includes a pediatric cardiologist, a pediatric cardiac echosonographer, and a specialized pediatric cardiac nurse. This multidisciplinary team travels to the child’s community hospital and collaborates with the community health team in the clinical assessment, treatment plan, education and support of each child and family.
The program services the following communities: Kamloops, Penticton, Vernon, Kelowna, Prince George, Terrace, Trail, Whitehorse, Victoria, and Williams Lake. Almost 600 children were seen in a partnership program clinic in 2004 with plans for continued growth in 2005.
Pediatricians can make referrals to the Provincial Partnership Clinic by contacting the clinic directly. Patient referrals can be faxed to Children’s Heart Centre with patient demographics, reason for referral, and any relevant background information. All referrals will be organized by priority with a cardiologist, and the partnership secretary will send a letter to parents if accepted. It may recommend that the patient be seen in Vancouver if space is not available.
