SPDC carries an extensive line of nutritional supplements and feeding-related supplies for individuals under the care of a physician or Registered Dietitian.

Discharging a Patient Home From Hospital?
Patients who need formula and/or supplies upon discharge from SPDC should be referred at least three working days before discharge to allow time for special items to be ordered.  In some cases, non-stock items can take three weeks or more to arrive in SPDC.  To fill out a referral form, please see below.
