Referrals
If the right services for your child are not available in your community, your
pediatrician
, family doctor, or, for some of our services, other health professionals may refer your child to Sunny Hill.
If your pediatrician or other health professional has questions about referring your child to Sunny Hill, he or she can call the Triage office at (604) 453-8320.
When we receive a referral, our
Triage
office decides which of our teams should see your child.
You will receive some important forms to complete. If you have any questions about the forms, please
call us
or talk to your pediatrician or other health professional.
Once you have returned the forms to Sunny Hill, we will contact you with an appointment date or to talk about which Sunny Hill services or other community services would best meet your child's needs.
Your child may be referred to one or more of the following programs at Sunny Hill:
Neuromotor
Neuromotor Physician Referral form
Autism and Complex Developmental Behavioural Conditions (CDBC)
Autism and CDBC Physician Referral Form
Shriners Gait Lab (Gait Lab)
Gait Lab Referral Form
Acute Rehabilitation (Acute Rehab)
Your child will usually come to Acute Rehab from another hospital. Physicians refer all children coming to acute rehab.
