Family care plan

The care plan and hospital registry were created by Partners in Care, the parent advisory committee to Children's. They can help to ease the transition for you and your family when your child is transferred from one hospital to another within the province.

Family care plan

When your child is being transferred to another hospital, use the care plan to communicate information that is unique to your child’s care. You will find a sample form for a care plan that you, the parent or guardian, and other members of the team from the transferring hospital can complete together. This will give the receiving hospital the information it needs to continue the care confidently. It will also provide a way for you to introduce your child to the new facility.

Hospital registry

The hospital registry was created to help you learn about the hospital that your child is being transferred to. By clicking on the name or unit of the hospital to which your child is being transferred, you will find a description of the hospital unit, its philosophy, layout and care capabilities.
