Connect4Care supports families by coordinating care for children

Through Connect4Care, a BC Children’s Hospital initiative, families have an active role as part of the team supporting their child; this helps improve access and promotes awareness of patient and family needs.  

Connect4Care was established after research with families showed there was a need for improved communication and collaboration between services. The new service now ensures families are involved in the planning of their child’s treatment and offers a single point of contact for a number of resources. Connect4Care focuses on children with complex medical, behavioural and developmental needs, with an emphasis on coordinating their care and supporting collaboration. 

The service continues to evolve as BC Children’s works with families to identify needs and co-design the service to reflect feedback from families. Connect4Care also has the ability to act as a bridge between the hospital and services within the patient’s home community.
Through Connect4Care, navigating services is much simpler, which means families can spend time where it matters most, with each other.
