1Fill in the referral form

If you want to fill in the form by hand, download the blank form, print it out and then fill it in.

If you want to fill in the form online, open the form, fill it in online, and then print it out. You can save the form to your desktop to fill in later.

2Include a growth chart and lab reports

Include a growth chart and any needed lab reports with your referral, and fill out the form completely. Missing information might cause a delay in scheduling an appointment.

3Fax the form

Fax the form to our office. Fax number: 604-875-3231. We will respond by fax within a few days to say whether the referral has been accepted, is pending receipt of more information, or has been declined.

4Let your patient know that we will contact them

If you have not already done so, let your patient know that we will contact them with an appointment time.
