Check in with the receptionist and wait for height, weight and temperature to be done. At this time we ask you to update your contact and primary physician information if changed.

You can expect your clinic visit to take around 2 hours to complete, as you will be seeing your transplant doctor and other transplant specialists.

The transplant nurse will first complete an assessment.

You will also see other members of the transplant team during a clinic visit (pharmacist, social worker, dietitian, psychologist, physiotherapist, research assistants, etc.).

You will be seen by your primary transplant physician. There may also be a fellow or other trainee that will see you before the physician.

Once the physician sees you, he or she will advise you of your next transplant clinic appointment, and when to complete your next bloodwork. You may be advised when you should arrange testing at the clinic lab, which includes the date of your annual testing.

We follow most patients every 3 to 6 months in the clinic depending on your child's age, but more frequently the first year after transplant. We will ask you to arrange a follow-up visit before you leave.

If needed, proceed to the outpatient pharmacy located on the ground floor of the Ambulatory Care Building to pick up your medications. Please remember to order your medications 1-2 weeks before coming to your transplant clinic appointment.

Take your morning medications after your blood work has been drawn. If you use a pill box, please bring it to clinic with you.

Prior to arriving for your scheduled clinic visit, we ask that you take a moment to complete any online questionnaires that we had arranged with you. These questionnaires give us additional information about you and your child's quality of life, mental health, readiness to transition to adult health care, and physical activity.

A summary of these questionnaires will be kept with the MOT team, and used to provide us extra information and to track improvements over time. Because of this, it is important that they are completed in a timely manner so we can review them in preparation for your visit.

For more information about our questionnaires, or for technical assistance, please discuss with any one of your transplant clinic team members.

If you are having your testing done on the day of your clinic visit, arrive at the lab on the ground floor of the Ambulatory Care Building between 7:30 and 7:45 a.m.

Identify yourself as a transplant patient to the receptionist and ask for your standing orders blood work to be drawn. You may also need to give a urine sample.

If you do your blood work the morning of your clinic appointment, the transplant nurse will call you in the afternoon, after your clinic visit, if there are any changes to be made to your medication doses.

Please confirm that you have received the nurse's message by calling the nurse coordinator (604-875-3604). Please leave a message if you call outside open hours.
