Hearing Loss Referral
Children and youth with all levels of hearing loss (and developmental questions/concerns) are eligible to see a developmental pediatrician and registered nurse.
Only those with permanent bilateral, moderate or a more severe degree of hearing loss are eligible for assessments from other team members. These may include developmental pediatricians, registered nurses, psychologists, speech and language pathologists, recreation therapists and social workers.
We will not accept referrals for psychoeducational assessment of children who have normal/slight-to-moderate hearing loss. We will redirect them to other services instead.
We require an audiology report for referrals to this service.

Complete a form
Please complete the referral form (PDF) in full and ensure that all patient information is current. Include consults, reports and/or chart notes (see referral form for requirements).
We will not process incomplete referrals.

If you have not already done so, let the patient and their family/caregiver know that we will contact them with an appointment time.
