Provincial Mental Health Metabolic Program Referral
This program provides care for children and youth with mental health concerns who are at high risk for metabolic side effects associated with taking certain types of psychiatric medications, such as second-generation antipsychotics (SGAs); or who have pre-existing metabolic conditions (excluding type 1 diabetes).
Eligibility criteria
Children and youth must have a mental health diagnosis and one component of metabolic syndrome (significant weight gain, high blood pressure, high cholesterol, high insulin, high blood sugar). We do not accept children and youth with diagnosed type 1 or type 2 diabetes.
Referral overview
We accept referrals from health-care providers such as community psychiatrists, pediatricians, family doctors or nurse practitioners.

Complete the referral form
Complete the Provincial Mental Health and Metabolic Program referral form (PDF). Include the requested assessment information noted on the form, including most recent height and weight, as well as mental health history.

Send the referral
Fax the completed form to 604-875-2271.

Post-referral
We aim to respond by fax within a week or two. We will indicate whether we have accepted the referral, require more information, or have declined it.

Appointment with a provider
If you have not already done so, please let your patient and their parent or caregiver know that we will contact them directly with an appointment date and time if we accept the referral.
