Gender Clinic Referrals

To refer a patient to the Gender Clinic, please follow the referral process and use one of the referral forms below.

1Fill in the referral form

If you want to fill in the form by hand, download the blank form, print it out and then fill it in.

If you want to fill in the form online, open the form, fill it in online, and then print it out. You can save the form to your desktop to fill in later.

2Include any notes from mental health providers and any significant lab reports

For treatment with blockers and/or hormones, the patient will eventually require an assessment to be completed by a trans-competent mental health professional (usually a psychologist or psychiatrist who has training and experience working with transgender and gender-questioning children/youth). If this has already been done, please forward a copy of that assessment to us, as well as any other significant reports from the patient’s mental health provider(s). If the patient hasn’t yet started the assessment process, we will assist them is getting this initiated if needed when we meet them for their first intake visit.

Please also include any relevant lab reports with your referral, and fill out the form completely. Missing information might cause a delay in scheduling an appointment.

3Fax the form

Fax the form to our office. Fax number: 604-875-2426. We will respond by fax within a week or two to say whether the referral has been accepted, is pending receipt of more information, or has been declined.

4Let your patient know that we will contact them

If you have not already done so, let your patient know that we will contact them with an appointment time.
